To help you better understanding your final notice for medical services rendered, we are including a numbered and annotated final notice sample that outlines the specifics for you.
SAMPLE
- Make Checks Payable To: make check payment to First Emergency Care Physicians, the Emergency Group that provided the service at the hospital at which you were seen. 
- Addressee: the addressee is the responsible party for the bill and services rendered. 
- Payment Options: there are four payment options available to you: 
 – securely pay online: https://www.patientonlinepay.com (formerly erstatement.com)
 – pay by phone: (800) XXX-XXXX 24-hours a day (sample phone # only)
 – pay by mail: mail payment with coupon, Client ID and Account # noted on check
 – quick pay by text: with Papaya by scanning the QR code by visiting ppaya/paynow
- Remit To: the address location where to send a check in the mail. 
- Email Inquiries: select the correct email below 
 – patient inquiries: patient@patientonlinepay.com
 – insurance inquiries: insco@patientonlinepay.com
 – attorney inquiries: chartswap.com
- Remit Payment: the address location where to send a check in the mail. 
- Patient Information: the patient who received medical tests and/or treatment services is listed, client number, patient account number and balance due. 
- Final Notice: a final notice about your bill, including the option of setting up a payment plan. 
- Insurance Notice: a final notice that the balance is what is remaining after insurance was applied.
